What Is the Transitional Housing Program?
This program is a collaboration between AHADC and Children and Youth Services of Dauphin County (CYS). The goal is to reduce the number of homeless families living in hotels/motels. Families remain unified while they gain the means to obtain permeant and safe housing of their own. This is a temporary program that is family focused through collaboration and communication by all parties involved, through a team effort. Ultimately working towards becoming self-sufficient with housing. This may be done through Public Housing, Voucher program, or independently paying.
AHADC will:
Purchase, furnish, and maintain these houses for families that are deemed homeless and have been living in hotels/motels.
Provide housing for the 12-month lease term which will be monitored month to month.
Determine if at 12 months, the family has not yet secured permanent housing of their own, but making progress, the lease can be extended up to 18 months.
Families that refuse to: participate, follow program policies/lease, or loose custody of their children, will be terminated from the program via 30-day eviction.
Provide frequent site visits to see condition of the home and review: housing safety/cleanliness, budgeting and paying bills/rent, how to obtain needed supports and services
·Communicate any concerns in regard to the safety and welfare of the children will be reported to CYS as well as concerns in regarding the family’s progress towards housing self-sufficiency.
Monitor and discuss the family’s needs for other services/providers and refer as needed.
Maintain a signed consent by the family to discuss pertinent information with other agencies, including CYS
AHADC will NOT:
Be responsible for all non-essentials and personal belongings.
Does not provide transportation or moving services.
CYS will:
Identify families to be referred to the program
Provide AHADC with full case management team for each referred family, including contact information
Pay rent due to AHADC on the 1st of each month for occupied homes based on the family income. CYS will pay 100% for a family with zero income or will pay the difference if the family has income to contribute towards rent, 30% or their adjusted monthly income will be determined as their rent share.
Communicate issues any needs or concerns with the family to AHADC in a timely manner.
Communicate if there is concern about the family’s ability to remain unified.
Notify AHADC any changes in the family unit be reported to AHADC within 24 hours.
